We invite you to join our Hematology/Oncology team as a Specialty Nurse Navigator.
As a Specialty Nurse Navigator, you will be responsible for:
v Formulating custom care plans from assessment and clinical information demonstrating the application of clinical guidelines including standards of care.
v Establishes specific, measurable, attainable, relevant and time specific nursing goals to guide the care planning process and promote self-reliance.
v Assists patients in establishing, modifying, or reinforcing a social support network, thereby promoting self-reliance. Identifies and aligns resources that impact high-risk care.
v Coordinates services on behalf of the high-risk patients.
v Work in collaboration with the Healthcare team to track and review data for research projects as well as resource utilization related to relevant specialty and make recommendations as needed.
Required Education/Experience/Specialized Skills:
* BSN plus 3 years recent clinical nursing experience in a variety of clinical settings.
Required Certification/Registration:
* CA RN License
* BLS if working in a clinical setting.
* ONS Chemotherapy and Biotherapy if working in Hematology.
Preferred Education/Experience/Specialized Skills/Certification:
* MSN
* OCN if working in Hematology ONN-CG if working in Oncology area.
* Two years experience managing care of relevant specialized patient population.
