JOB SUMMARY
Jacob A. Riis Neighborhood Settlement NNORC Program is actively seeking a Bilingual Case Manager to join our dedicated team. In this role, you will play a pivotal part in enhancing the quality of life for Queensbridge Older Adults aged 60 and over by providing case management services. We aim to empower these individuals to lead healthier and safer lives within their own community, fostering successful aging in place. The core services you will provide encompass comprehensive assessment and re-assessment, care planning, coordination of services, follow-ups, and monitoring. Additionally, you may engage in counseling, advocacy, and assistance with paperwork and applications for benefits and entitlements. Routine home visits will be a crucial aspect of your responsibilities. As a Bilingual Case Manager, you will cultivate and leverage relationships with service providers and referral sources to ensure that our older adult participants receive the support they need. Collaborative case management with fellow Case Managers is a key component of this role, facilitating holistic care for participants within the NNORC Program.
JOB RESPONSIBILITIES
Major Duties and Responsibilities
- Liaise with Older Adult staff to secure referrals and confirm client appointments, fostering connections with client and community resources.
- Collaborate closely with clients, their families, healthcare providers, and community partners to assess needs, craft care plans, facilitate resource access, and adapt plans as required.
- Engage in shared case management alongside on-site nurses, occasionally accompanying them on visits.
- Efficiently coordinate and oversee support groups, ensuring their smooth operation.
- Maintain comprehensive daily, weekly, or monthly case reports for clients.
- Serve as a liaison between clients and other agencies, aiding in communication with clients and their families.
- Thoroughly document all direct and collateral interactions, diligently updating case files.
- Adhere to meticulous documentation procedures, guaranteeing completeness and accuracy of case notes and charts for quality assurance.
- Conduct bi-annual chart reviews to uphold quality standards.
- Actively participate in agency activities, including meetings, outreach initiatives, fundraising, case manager training, and special projects as directed by the NNORC Program Director.
- Flexibly to assist with various program components as needed.
EDUCATION AND EXPERIENCE:
- Preferred: Master's Degree in Social Work; Bachelor's Degree in Social Work, Human Services, or related field with three years of experience considered.
- Strength-based training such as Family Development Credential
- Experience with Older Adults or understanding of Older Adults' unique needs.
- Previous work with non-profits or community organizations.
- Proven experience in comprehensive case management of targeted population.
- Comfort and experience with home visits in NYCHA (New York City Housing Authority) public housing developments
