General Job Description
*As a Case Manager-Prenatal for Family Health Centers of San Diego, you support our Prenatal patients by coordinating patients' care plans. Case management includes client enrollment, orientation, patient assessment, appointment follow-ups, and ensuring documentation is complete and accurate. You support the mission of FHCSD by ensuring our Prenatal clients and their families experience excellent customer service and fully utilize available services. You have a passion for community health care, for serving the medically underserved, and you understand how important quality prenatal care is for both mothers and infants.
Job Roles
*Assists in patient flow. Ensures that test results and referrals are given to appropriate team members and accurately recorded. Reviews medical records prior to appointments to ensure completion and accuracy.
*Completes all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards. Ensures that test results, referrals, and reports are given to appropriate team members for review and are accurately recorded. Reviews medical records prior to appointments to ensure completion and accuracy. Scans various medical reports into electronic health records following established protocols to ensure information is available through hospital portal at time of delivery.
*Coordinates client's individual care plans and oversees completion of all recommendations made on the care plan. Assists clients with practical arrangements such as translation needs, referrals, special appointments, Medi-cal eligibility, and utilization of internal and external services (e.g., HIV testing, pediatrics, speech & hearing).
*Ensures cases are closed following established standardized protocols.
*Keeps prenatal logs (pregnancy testing, master list, abnormal pap, etc.) current and provides appropriate and timely follow-up.
*Performs other duties as assigned.
*Provides individual client orientation, education, combined assessments, and reassessments as needed. Works closely with team, Lead, and/or Supervisor to meet department productivity goals.
*Provides pregnancy testing and counseling to clients in an unbiased manner, providing appropriate referrals.
Education/Certifications/Licenses/Registrations
*1 year experience in perinatal or women's health care preferred.
*Bachelor's degree in Health Science or Social Science field preferred.
*High School diploma or GED equivalent required.
*Medical Assistant diploma or LVN desirable.
Experience/Specialized skills (including Language)
*Ability to accurately and quickly perform data entry.
*Ability to work effectively with patients from diverse social, cultural and economic groups.
*Ability to work well in both a team-based environment and independently.
*Bilingual in Spanish/English required.
*Demonstrated ability to be culturally sensitive and respect diversity.
*Good written and verbal communication skills.
*Knowledge of perinatal programs, care management, and procedures.
*Strong computer literacy ability to comply with department needs and expectations (i.e., electronic medical record documentation, obtaining background information and reports on patients, following up on appts, etc.).
*Strong interpersonal and customer service skills.

$21.00 - $26.88If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
