Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.
Shifts Available:
Full Time 40 Hours per Week | 8:00am - 4:30pm | Monday - Friday with rotating crisis cell phone during after hours
Expected starting wage range is $19.90 - $26.00. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.
What You Bring to the Table (Must Have)
- Bachelor's degree in Social Services or Human Services and (1) year of experience or High School Graduation or G.E.D. equivalent and four (4) years of direct services in Mental Health or community services; an Associate's degree may be substituted for two (2) year of experience.
- Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
- Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
- Willingness to use the company vehicle to drive clients to appointments and groups, etc.
- Willingness to use your personal vehicle to attend meetings, etc.

Who We Serve: Adults and transitional aged youth (TAY) with serious mental illness who have recently been discharged from county-identified long-term care facilities.
About the Program:
At Pathways to Recovery, we believe in a person's right to full inclusion and to a meaningful life of their own choosing, free of stigma and discrimination from the moment you walk in the door. Our program provides psychosocial rehabilitative, recovery-oriented, intensive, community-based services to help members navigate the mental health system and take positive steps toward their recovery. In-Reach is a sub-component of PTR and works in conjunction with San Diego County Institutional Case Managers (CICM) to provide case-aide services that are focused on discharge planning and benefit coordination for CICM members residing in long-term care.
